Leave in Term Time Form

All leave in term time MUST be approved by our Head Teacher and by completing this electronic form you are ensuring that all information is shared as efficiently as possible.  Before clicking on the link please ensure you have to following information - 

• Childs Name
• Childs Class
• Dates of Travel
• Number of 'school days' the child/children will not be in school.
• Address of accommodation where you are staying
• Evidence of travel 
• Evidence of Accommodation 

Once the form is submitted the school will be informed via email so as approval can be requested from the Head Teacher. 

You can also download the form (below), print it and complete it manually showing evidence of travel and accommodation in the school office.
